History & Mission

Community Health Centers, Inc. (CHC) is not just a healthcare provider; it’s a beacon of hope and compassion in the community. Founded on the belief that everyone deserves access to quality healthcare, regardless of their ability to pay. CHC has been serving vulnerable and underserved populations since 1978.

At the heart, CHC’s mission is a commitment to patient-centered care, where every individual is treated with dignity and respect. The diverse and dedicated team at CHC works tirelessly to break down barriers to healthcare, whether financial, cultural, or logistical, ensuring that no one falls through the cracks.

In 2023, CHC proudly became the first Teaching Health Center GME (THC-GME) in Utah, furthering CHC’s commitment to medical education and training the next generation of providers.

Program Overview

Our curriculum is tailored to foster full-spectrum family medicine physicians. Residents gain valuable experiences through inpatient mini-blocks and longitudinal rotations in ambulatory pediatrics, women’s health, sports medicine, and procedure clinics at a federally qualified health center. With the flexibility to customize their electives, residents have the opportunity to delve into areas of personal interest and professional growth. 

Program Aims

  • Recruit, support, and retain a diverse cohort of residents reflective of our communities that fosters an inclusive learning environment that celebrates individual differences and perspectives.
  • Enable resident physicians to develop proficiency in the ACGME core competencies in order to practice autonomously, providing high-quality, patient-centered care.
  • Address healthcare disparities including access to care and social determinants of health by receiving core residency clinical training including the family medicine continuity clinic located within a Federally Qualified Health Center.
  • Foster clinical excellence through rigorous training in evidence-based medicine, procedural skills, and comprehensive primary care to ensure that our residents graduate with the clinical proficiency necessary to excel in any healthcare setting, with an emphasis on rural and underserved populations.
  • Actively engage with local organizations, healthcare providers, and community leaders to foster partnerships that enhance patient care, expand access to services, and address the needs of our communities.

Welcome to the CHC Family Medicine Residency Program

First off, congratulations on your accomplishment: you are a physician! You have embarked on a career dedicated to delivering compassionate healthcare, working collaboratively with others, serving your community, and exercising leadership to make a meaningful impact. We are honored to participate in your journey and thank you for choosing to learn more about us.

At CHC Family Medicine Residency, we are committed to providing an exceptional educational experience that combines rigorous clinical training with a supportive and collaborative environment. Our goal is to develop well-rounded, compassionate family physicians who are equipped to meet the diverse needs of our patients and communities.

As a resident, you will benefit from a comprehensive curriculum that includes diverse clinical experiences, from primary care and preventive medicine to acute and chronic disease management and care of young families including pregnancy care and pediatrics. Our curriculum is focused on clinic first supported by our mixed mini-block (two week) and longitudinal model. You will work closely with a talented and dedicated team of faculty and healthcare professionals who are passionate about teaching and patient care. You will be a valued member of our healthcare team at the CHC actively participating in our mission to improve the health and wellness of our community.

As a Teaching Health Center, our program differs from traditional residencies in that we are based at and governed by the Community Health Center rather than a large hospital or academic medical center. This enables us to create resident education specific to patient care in our community and meet the needs of both the patients and resident learners. This includes training in pregnancy-related care, behavioral health, social determinants of health and language (proficiency in Spanish). This also allows residents to train in settings in which they will work in the future with medically underserved populations at ambulatory clinic at the CHC and inpatient at the affiliated urban community hospital, Intermountain LDS Hospital. Our faculty are experienced teachers of residents in OB-GYN, Family Medicine and Pediatrics from the University of Utah as well as medical students and Advance Practice Clinician students. We have run the Family Medicine Obstetrics Fellowship for over 20 years.

Our affiliation with Community Health Centers (CHCs) underscores our commitment to serving underserved and diverse populations. CHCs are vital to our healthcare system with one in 11 people in the United States receiving primary care from CHCs. We provide accessible, high-quality care to communities that might otherwise face barriers to receiving medical services. Here, you’ll gain hands-on experience in a setting that emphasizes the importance of comprehensive, patient-centered care and community engagement. Your work will directly contribute to improving health outcomes and reducing health disparities, aligning with our mission to make a meaningful impact in the lives of those we serve. Our Community Health Center was created over 40 years ago in Salt Lake County and has been a leader in care of the underserved, primary and preventive care, pediatric and pregnancy-related care. We have demonstrated resiliency through health, political, financial and pandemic-related challenges in 6 different decades as our community in Utah has grown.

From hiking and skiing to exploring the national parks and enjoying the vibrant local culture, you’ll find countless opportunities to enjoy the natural beauty and recreational activities the state has to offer. Our residency is located in Salt Lake County home to Salt Lake City one of the largest cities in the Mountain West with an international airport allowing travel to anywhere. Most important for the busy resident physician is the accessibility of it all, literally out your door.
